Re-Entry Services

The purpose of re-entry is to assist inmates to prepare for their transition from jail back to the community by developing a re-entry plan to minimize the risk the inmate will return to jail. The plan is developed around identified barriers to successful reintegration.
These barriers include:
- Housing
- Family issues
- Employment
- Education
- Transportation
- Legal issues
- Mental health
- Substance abuse
- Peer association
- Attitude toward crime
In conjunction with personal interviews with the inmates, the CJS re-entry case manager also gathers additional corroborating data. In subsequent meetings with the inmate, a plan is developed to address treatment and services needed to address the inmates identified barriers. As part of the plan, specific community and correction resources are identified to provide the post-release needs of the inmate. Upon release, the re-entry case manager coordinates with the inmate and the identified resources to ensure implementation and avoid gaps in the services. The CJS re-entry case manager works with the individual up to 90 days after release.
